Brien O'Keeffe, a watch manager who ran the bridgehead during the early stages of the Grenfell fire, describes the rapid escalation from a single flat fire to a building-wide catastrophe. His testimony reveals critical moments when firefighters realized compartmentation had failed, when burning debris began falling from the building, and when the 'stay put' advice became untenable as smoke filled stairwells and residents fled in panic.
